We process the personal data obtained for the purpose related to your participation in the 1st Physique Sports Festival – Warsaw 2020.
The legal basis for processing your personal data is: - Your consent to the processing of personal data contained in the declaration form (Article 6(1)(a) GDPR)
- Your consent to the use of image for purposes related to your participation in the 1st Physique Sports Festival – Warsaw 2020 (Article 9(2)(a) GDPR)
- Our legitimate interest – organization of the 1st Physique Sports Festival – Warsaw 2020 (Article 6(1)(a), GDPR)
The recipients of your personal data will be competition organizers, judges, other competition participants.
The organizer is obliged to transfer your personal data outside the EEA (covering the European Union, Norway, Liechtenstein and Iceland) to the NPC Worldwide / IFBB Professional League having its registered office in the USA and granting the organizer a license to organize the competition as a part of the 1st Physique Sports Festival.
Your personal data will be kept by the organizer for a period of 10 years after the declaration has been signed.
Your rights related to the processing of personal data:
You can submit an application to the Organizer regarding personal data for:
- rectification / correction of personal data (Article 16 GDPR);
- deletion of data (Article 17 GDPR);
- limitation of personal data processing, i.e. suspension of operations on personal data or non-deletion of data – pursuant to the submitted application (Article 18 GDPR);
- access to personal data (for information about data processed by this entity and for a copy of the data – Article 15 GDPR)
You have the right to lodge a complaint with the President of the Personal Data Protection Office if you believe that the processing of personal data violates the law.
The provision of personal data by you is not a statutory requirement. If you fail to provide personal data, you will not be allowed to participate in the 1st Physique Sports Festival – Warsaw 2020.
Your personal data will not be subject to automated decision making processes by the Organizer, including profiling.
Please be advised that pursuant to Article 7(3) GDPR, you can withdraw your consent to the processing of personal and biometric data by the Akademia Sportów Sylwetkowych at any time. Withdrawal of the consent does not affect the lawfulness of the processing performed based on the consent before its withdrawal.

PARTICIPANT’S DECLARATION
I, the undersigned, declare that my health allows me to participate in the 1st Physique Sports Festival – Warsaw 2020 – in the shows and sport competition. I participate in them voluntarily and on my own responsibility. At the same time, I declare that I have read the rules of the competition and I fully accept the provisions contained therein.
In addition, I agree to publicly disclose my name and surname during the 1st Physique Sports Festival – Warsaw 2020, as well as to their publication in the rosters and reports of the 1st Physique Sports Festival – Warsaw 2020.
PARTICIPANT’S DECLARATION
I consent to the processing of my personal data and my biometric data (physical likeness) by the Akademia Sportów Sylwetkowych for the purposes necessary for the organization of the 1st Physique Sports Festival – Warsaw 2020, in accordance with Article 6 (1)(a) and Article 9(2)(a) of the Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC (General Data Protection Regulation).

AUTHORIZATION (CONSENT) TO DISSEMINATE THE IMAGE
Pursuant to the provisions of Article 81 of the Copyright and Related Rights Act of 4 February 1994 (Polish Journal of Laws of 2017, item 880, as amended), I authorize the dissemination of my image by the owner of the Akademia Sportów Sylwetkowych, with the registered office at ul. Szwoleżerów 128 lok. 64/65, 05-091 Ząbki, hereinafter referred to as the “Organizer”, for promotional, information, cultural and educational purposes related to the tasks carried out by the Organizer. The consent to the dissemination of my image includes in particular such forms of its publication as:
1) sharing on the websites: www.ifbbpro.com.pl , www.festiwalsportowsylwetkowych.pl
2) placing in promotional and informational materials (in audio-visual materials, brochures, newspapers, on bulletin boards, in connection with participation in competitions and other forms of cultural and educational activity),
3) sharing photos of me in electronic form in social networking platforms (Facebook, Instagram) for promotion of the Physique Sports Festival,
4) I consent to the image processing by its cropping and composition. The image may be used in accordance with the principles set out above for a period of 10 years after the consent has been signed.
