CONSENT TO THE USE OF IMAGE

1. I hereby give my consent to use of the image of me by Akademia Sportów Sylwetkowych with the registered seat in Ząbki, entered in the CEIDG [Central Registration and Information on Business] under No. 000000542/2018 (hereinafter referred to as the Akademia) for advertising, promotional, information and marketing purposes related to the business operation conducted by the aforementioned company aimed at organizing competitions in physique sports and promoting these sports.
2. I give my consent to use the image of me by the Akademia or by other persons acting on behalf of the Akademia or in consultation with the Akademia, including its publication, dissemination, recording and distribution of copies on which this image has been recorded, as well as reproduction and duplication, processing (retouching), cropping and any composition of the image with the use of all currently available techniques and methods (including digital techniques and methods), also together with images of other people recorded as part of achieving the objectives referred to in item 1 herein. The areas of the use include:
   a) electronic media, in particular on the following websites: www.ifbbpro.com.pl, www.festiwalsportowsylwetkowych.pl and in social networking platforms;
   b) press and television;
   c) advertising and information materials, in particular in brochures, leaflets, posters, postcards, newspapers, calendars, folders, etc.
3. This consent is not limited in time or with respect to territory.
4. I declare that the use of the image of me, in accordance herewith, does not infringe anyone’s personal rights or other rights.
5. The Akademia may transfer (including a transfer for a fee) the right to use the image to other entities on the terms and conditions set out in items 1 and 2 herein.
6. I declare that I give this consent free of charge.
